CHCSS00099 - Individual Support – Home and Community (Ageing) Skill Set

This skill set reflects the skill requirements for providing person centred support to older people in home and community settings. This skill set is for people working in various care and support roles in the community services sector who wish to develop skills in home and community support.

This skill set reflects the skill requirements for providing person-centred support to older people in home and community settings.

 

Course Duration: 8-9 months

Learners are given 8-9 months to complete all learning and assessments.


Study Commitment
: 10 hours per week

It is anticipated that a learner would need to dedicate 10 hours per week. This time may include:
- Classroom training
- Assessment support sessions (online or in the classroom)
- 1:1 Coaching sessions
- Completing assessments
- Self-directed learning activities
- Self-directed research
- Work placement


The units that this skill set covers are:

CHCAGE001 Facilitate the Empowerment of Older People
This unit describes the skills and knowledge required to respond to the goals and aspirations of older people and provide support services in a manner that focuses on improving health outcomes and quality of life, using a person-centred approach This unit applies to support workers in residential or community contexts.

CHCHCS001 Provide Home & Community Support Services
This unit describes the skills and knowledge required to work in a home support environment and community settings with individuals, family members, staff, visitors, suppliers and others to meet established work requirements. This unit applies to workers in a community services context. Work performed requires some discretion and judgement and is carried out under regular direct or indirect supervision.

CHCCCS011 Meet Personal Support Needs
This unit describes the skills and knowledge required to determine and respond to an individual’s physical personal support needs and to support activities of daily living. This unit applies to workers who provide support to people according to an established individualised plan in any community services context. Work performed requires some discretion and judgement and may be carried out under regular direct or indirect supervision.

CHCCCS025 Support Relationships with Carers and Families
This unit describes the skills and knowledge required to work positively with the carers and families of people using the service based on an understanding of their support needs. This unit applies to workers across a range of community services contexts.

  • Be the minimum age of 18 years old

  • Be proficient in the English language, with a good standard of literacy and numeracy skills

  • Have access to internet, webcam, email and MS Word, Adobe software and computer no older than 5 years

  • Complete a Language, Literacy and Numeracy Evaluation

  • Have a current National Police Check

  • Have a NDIS Workers Screening Check (required by some workplaces)

  • Have a current Working with Children Check (required by some workplaces)

  • Maintain reasonable physical health and fitness as the role involves a variety of physical tasks, including providing direct personal care including manual handling and other physically demanding activities. Students also require a level of maturity with a genuine interest in working with older people.

  • Classroom Training – 8 x 8-hour classroom session. Learning activities will reinforce theoretical knowledge, and workplace skills will be practiced and placed into context.

    Assessment Support Sessions – Assessment support sessions will be offered to support learners in the completion of their assessments. Learners will be invited to attend either face to face sessions or online support sessions.

    Coaching Sessions – 1:1 Assessment Support sessions are scheduled, as required with up to 2 x1 hour session per learner over the duration of the course.

    Self-paced Learning – Learners are encouraged to undertake 10 hours per week of self-paced learning throughout the duration of the course. This time is allocated to review their learning for the current and upcoming weeks and undertake research to prepare responses to research-based assessment tasks.

    Work Placement – A minimum of 40 hours of work placement is undertaken throughout the duration of the course. This time may include classroom training, assessment support sessions (online or in the classroom), 1:1 coaching sessions, completing assessments, self-directed learning activities, self-directed research and work placement.

    Workplace Observation – The Trainer/Assessor schedules at least one visit to each learner at their work placement location, where practical demonstration is observed and assessed.

    This skill set reflects the skill requirements for providing person centred support to older people in home and community settings.

    These units provide credit towards a range of qualifications in the CHC Community Services Training Package at Certificate III or Certificate IV level. Upon completion of this qualification, students may enrol into: CHC33015 Certificate III in Individual Support.

    RPL

    Students who believe they are already competent in parts of the course may apply for RPL. An application for RPL may only be made after enrolment and must be made using the RPL Application Form that will be provided. RPL is a form of assessment and requires the same application of the principles of assessment and the rules of evidence which are applied in any form of competency-based assessment.

     

    Credit Transfer

    Students who have completed units of this course at other registered training organisations will be given a credit transfer on presentation of a verified transcript, award or Statement of Attainment.
